We offer two options for completing the Credit Application and the Order Form should you decide not to order via our e-commerce website. The first option for your convenience is to download, print and return via the fax (763) 263-8577 or mailing them at your convenience. The forms listed below are in Adobe Acrobat pdf file format and Microsoft Excel.

If you do not have Adobe Acrobat loaded on your computer, you can download it free by clicking on the icon below. The second option is to copy the Excel file we have provided to your computer and then complete the forms and return them to Alpha Medical Equipment, Inc. as an attachment via the e-mail to or by mailing them to us at the address listed below.
